We highly recommend sending an email to your team(s) to remind them to complete the survey. People are often more likely to open an email from a name they recognize. Feel free to utilize the following suggestion and modify it to tailor it to personally address your team.

We're asking for your participation in completing the Team Member Perceptions of Instructional Change Team Collaboration (TM-PICC) survey. This survey is an important tool to help us better understand and improve how we work together as an instructional change team.
